If a hurricane is coming you want to have a plan already in place to care for you pets and keep them safe. Hopefully, you will never need to use it, but having it in place could save your pet's life and help you reunite if you are separated.
Always keep your pet's vaccinations current, but keep the documentation in a place where you can access it easily if needed. If you are issued an order of evacuation, you will be able to to right to the documents and have it ready for the shelter.
Keep a current photo of your pet with you. If your home becomes flooded, and images you had of your pet may become damaged or lost. Having their picture on you means that you can be reunited with your pet more easily if you become separated.
Even if your pet has a ID chip, it is still a good idea to have a collar with an ID tag. This will make it easy for anyone who finds your pet to contact you.
Many areas now have shelters for pets, in case of an emergency. Hurricane Katrina taught us that it is important to provide shelter for them too. Check with your vet or local animal shelter for instructions of where to go in your area.
Make sure you have a pet carrier, in the proper size for all of your pets. This will help to keep them calm and contained while transporting them to the shelter
Bring all your pet's documentation, along with food and water to the shelter. Call ahead to make sure the shelter is not already full. Most won't reserve a spot, and will only take who gets there first.

I fully expect to be living near Key West in about 10 years and these ideas are so vital to making sure my pets are safe. I would NEVER EVER leave my pets behind; being prepared for bad weather is key to their, and my, health!
